Dear Tucson conference participant,

Thank you for taking part in the 2010 "Toward a Science of
Consciousness" conference.

We would greatly appreciate receiving your feedback on the conference.
This information will be very helpful in planning future meetings.
Please fill out this form and return it by e-mail to
center@u.arizona.edu.  If you did not take part in the conference,
then please ignore this e-mail.

If you choose, you can remain anonymous, so that identifying
information will be stripped before your answers are circulated to the
conference program committee. In any case, your answers will not be
circulated beyond the committee.  You are welcome to answer only some
of the questions if you prefer.
